Testing the Integrity of a Roux-en-Y Hepaticojejunostomy: The Air Insufflation Test
Hepaticojejunostomy (HJ) is a technically challenging surgical procedure especially in setting of benign biliary stricture or thin walled duct in choledochal cyst. Despite refinement in technique over period of time, post operative bile leak occurs in 2.3-12% of patients1,2. Leak following HJ can have varied consequences ranging from small self limiting bile leak to persistent high output external biliary fistula leading to sepsis, fluid electrolytes and nutritional problems, severe morbidity and even mortality.
